In-Network Plans
At this time I accept the following insurance plans:

- UnitedHealthcare

- Horizon Blue Cross Blue Shield (New Jersey)
Using Other Insurance
I am considered out of network for all other insurances. You will be responsible to pay my rate out of pocket for each session (charged at the session) and then submit a receipt (superbill) to your insurance company.
I will provide a monthly receipt (superbill) upon request so that you can submit to your insurance company. You may be entitled to out-of-network reimbursement (full or partial) based on your plan. Please check with your insurance provider prior to our first session so you are fully aware of your out of network benefit.
How It Works
I utilize a HIPAA compliant platform both for insurance clients as well as for private pay clients. You may use any major credit card when paying for the session. I will ask you to place a card on file for co-pays and for private pay fees.
All new client paperwork will be sent to you electronically and needs to be completed no later than 24 hours prior to our first session.
Cancellation Policy
I require that you provide me with 24 hours notice if you need to cancel. Late cancel and no-show fee is charged at a rate of $65.
